Students must be 4 years old by September 1 in order to enroll in our Prekindergarten program.  

 

We ask the family to provide the following documents:

• Birth Certificate (copy)

• Baptismal Certificate (only if Catholic) 

• Student Registration Form

• Family Registration Form

• Authorization for Care Form

• Immunization Records

• Medical Form (signed by Physician or Licensed Practicioner)

• Lunch Program Application
